How to Request a Sick Note:
To extend an existing sick note:
Fill in an ADMIN query form on Systm connect, available Monday to Friday from 8am. We will reply within 24–48 hours during our opening times.
To request a new sick note:
A clinical review is needed. Please fill in a CLINICAL query form on Systm Connect, available Monday to Friday from 8am. We will reply within 24–48 hours during our opening times.
Alternatively, you can call or visit any of our four surgeries between 8.15am and 6pm, Monday to Friday.
